USS ASHLAND (LSD-1) COMMISSIONED, 29 NOVEMBER 1961
12-16 Moored port side to the north side of pier 14, U.S. Naval Ampbibious Base, Little Creek, Virginia. Receiving the following services from the pier: telephone services. Ships present include various units of the U.S. Atlantic Fleet, yard and district craft. 1345 Officers and crew assembled for commissioning Ceremony. 1415 Vice Admiral Alfred G. WARD, USN, and party, came to the pier to commission USS ASHLAND (LSD-1). 1500 Pursuant to BuPers Order 104927 of 19 September 1961, Commander Arthur W. Price, Jr., 430937/1100, USN, Prospective Commanding Officer, read his orders, assumed command of ASHLAND (LSD-1), and ordered the commission pennant broken, the ensign hoisted, and the watch set. 1503 Vice Admiral Alfred G. WARD, USN, COMPHIBLANT, broke his flag in this vessel. The following officers and enlisted personnel were aboard at the time for the commissioning: See attached list of personnel on board. 1533 Vice Admiral Alfred G. WARD, USN, and party left the pier.

SOPA is CTF 61 in USS CAMBIA (APA-36). 0800 The Commanding Officer, USS CASA GRANDE (LSD-13), with his Executive Officer and party came aboard. 1100 The Commanding Officer, USS CASA GRANDE (LSD-13), with his Executive Officer and party left the ship. 1100 Commander, Amphibious Squadron Four relieved Commander, Amphibous Squadron Six as Commander, Task Force 61 on board USS FREMONT (APA-44).

12-16 Anchored as before. 1240 Pursaunt to orders of the Commanding Officer, Ltjg Julius N. Balog, USNR departed for temporary additional duty as Shore Patrol Officer. 1500 Set the Anchor Detail. Commenced maneuvering at various speeds to form lee. 1544 Secured the Anchor Detail.
16-20 Anchored as before. 1640 WARNER, D.(n), SA, of ACB-2 was injured when hit bay another man with a coffee cup receiving a wound, lacerated, occipital regaion, no A or N involvement. Treatment: Cleansed, three (3) 4-0 black silk sutures, dry dressing. Witnesses: None. Disposition: Returned to duty.

Test: Satisfactory. 1012 While scrapping lye from the bottom of a bucket, GUISBERG, L.(n), MM1, USN, received a burn of both eyes when lye flew into his eyes. Treatment: Thoroughly flushed with water, normal saline and diluted vinegar. Boric acid ophalmic ointment applied and dark glasses. Disposition: Limited duty for 24 hours.
